Job Summary

Trinity Wound Care is seeking a highly skilled and compassionate Wound Care Nurse Practitioner to join our dynamic healthcare team in Austin, TX. The ideal candidate will have a strong foundation in wound care management and thrive in a collaborative, patient-focused environment. This position offers the opportunity to make a significant impact on patients' lives through individualized, evidence-based care across a variety of clinical and home-based settings.

Key Responsibilities

• Conduct comprehensive assessments for patients with both acute and chronic wounds.
• Develop and implement personalized treatment plans based on clinical findings and best practices.
• Perform advanced wound care procedures including debridement, dressing changes, and related interventions.
• Educate patients and caregivers on wound care techniques, prevention, and self-management strategies.
• Accurately document all encounters using appropriate ICD-10 coding and maintain comprehensive medical records.
• Collaborate with an interdisciplinary healthcare team to coordinate care, with a focus on patients managing diabetes, peripheral vascular disease, or other complex medical conditions.
• Travel to various care settings, including patients' homes, Skilled Nursing Facilities (SNF), and Assisted Living Facilities (ALF) as part of mobile healthcare delivery.
• Participate in ongoing professional development and remain current on best practices and innovations in wound care.

Qualifications

• Valid and current Nurse Practitioner OR Physician Assistant license (State of Texas).
• Experience in Wound care settings is highly preferred.
• Proficient in wound care management, including debridement techniques and related procedures including wound vacs.
• Familiarity with ICD-10 and wound care coding standards.
• Background or experience in surgery or working with vascular patients is highly desirable.
• Strong interpersonal and communication skills with a patient-first mindset.
• Willingness and ability to travel to multiple care sites throughout assigned area.
