Position Title:Wound Care Nurse - Local TravelLocation: Austin, TXEmployment Type: Local Travel/PRNPosition Overview:MLee Healthcare has partnered with a state recognized home healthcare provider who is rapidly expanding their Austin market. We are looking to establish a team of motivated, and qualified nurses to respond within 24 to 48 hours of a patient referral. The RN will be responsible for providing high-quality nursing care to patients in their homes. The role includes patient admission, assessment, care planning, and ongoing follow-up visits to ensure optimal patient outcomes.Key Responsibilities:Patient Admission and Assessment:Conduct initial patient assessments and health evaluations.Develop and implement individualized care plans based on patient needs.Educate patients and families about care plans and health management.Care Coordination:Collaborate with multidisciplinary healthcare team members to ensure continuity of care.Coordinate and communicate with physicians, therapists, and other healthcare providers.Adjust care plans as necessary based on patient progress and feedback from the healthcare team.Patient Care and Management:Provide direct nursing care, including medication administration, wound care, and other skilled nursing tasks.Monitor and document patient health status and progress.Ensure patient safety and comfort at all times.Weekly Follow-Ups:Conduct weekly follow-up visits to assess patient progress.Update care plans and interventions as needed.Provide ongoing education and support to patients and families.Compliance and Documentation:Maintain accurate and timely patient records in compliance with healthcare regulations.Ensure adherence to all healthcare policies and procedures.Participate in quality improvement initiatives and audits.Qualifications:Education:Associate or Bachelors Degree in Nursing from an accredited program.Licensure/Certification:Current and valid RN license in the state of practice.BLS certification required; ACLS and other certifications preferred.Experience:Minimum of 2 years of nursing experience, with a preference for home healthcare or hospice experience.Strong clinical skills in patient assessment, care planning, and management.Skills:Excellent communication and interpersonal skills.Ability to work independently and manage multiple patients.Proficiency in electronic medical records (EMR) system
